Total hip replacement is a reliable treatment for advanced hip arthritis and other conditions causing severe pain and loss of function. The surgical approach used can influence soft-tissue preservation and early recovery. SPAIRE is a modern tendon-sparing modification of the posterior approach developed at the Exeter Hip Unit, where Dr Woodbridge completed fellowship training in hip arthroplasty.
What is the SPAIRE Technique?
SPAIRE stands for Saving Piriformis And Internus, Repair of Externus.
In a conventional posterior approach several short external rotator tendons are detached and later repaired. SPAIRE develops a lower interval so that the piriformis and obturator internus tendons remain intact. Only the obturator externus tendon is divided and repaired together with the posterior capsule. This aims to preserve more of the natural soft-tissue stabilisers of the hip while still allowing safe surgical access.
Potential Benefits
Where clinically suitable, the SPAIRE approach is designed to:
- Maintain key posterior stabilising structures
- Support early mobilisation and confidence with movement
- Reduce the need for traditional hip precautions in many patients (advice is always individualised)
Recovery still depends on overall health, pre-operative function, implant positioning and rehabilitation. SPAIRE is one of several contemporary options; the most appropriate technique is selected after clinical and radiographic assessment.
Recovery Overview
Most patients begin walking with assistance on the day of surgery or the next day. Physiotherapy focuses on gait, muscle activation and progressive independence. Walking aids are typically used for several weeks, with gradual return to daily activities and later higher-demand pursuits under guidance. Individual timelines vary.
